Living and Dying on the Streets: The Social Determinants of Palliative Care – Webinar – 2021-03-16

Dr. Trevor Morey is a palliative care physician with the PEACH team (Palliative Education and Care for the Homeless) an outreach team that provides palliative and end of life care for people experiencing homelessness in Toronto, Ontario. He also is passionate about advocating for improved health care and social services for structurally vulnerable groups. He is the communications lead for Health Providers Against Poverty and is a member of a working group developing a palliative care strategy for Corrections Services Canada. He uses the preferred pronouns he/him.

Leeann Trevors is a Registered Social Worker committed to an anti-oppressive and client-centered
practice. With over ten years of volunteer and work experience in the violence against women (VAW),
palliative care, and housing sectors, Leeann has worked with diverse populations in community and clinical settings. Following the completion of her Master of Social Work degree in Spring 2020, Leeann began working with the PEACH (Palliative Education and Care for the Homeless) Program through the Inner City Health Associates. She is dedicated to upholding client dignity and improving client outcomes through interprofessional collaboration and compassionate care.
